Please try returning to the forms download on the website, complete your details and select “Run” (rather than “Save”).
I want to install the forms across a network - ie NOT on a C drive
This will require a manual installation to the loaction where you keep your shared templates. You may download a zipped file of the forms here. (zip file 2.2 MB) Extract the files and save them to your preferred directory location. Staff will open them using File New as with any other template they currently use.

No. We can now only support the forms on systems with Microsoft Office Word 2000 or later

On some systems the smiley face will only appear when you are actually working on a form. If the smiley face still does not appear you can use the following key strokes to effect the recalculation – Ctrl and A together then F9.
No drop down menu appears
The drop down will only appear if you are on a single computer using a C Drive.
NB: If you want to install the forms on a shared directory, you will need to manually install them - but you will not have the drop down menu feature
- In Word, look under Tools>Options>File Locations to see where the Startup folder is pointing. NOTE you will not see the “LSANormal.dot” file.
- Using Windows Explorer ensure that the “LSANormal.dot” file is in the Startup folder that you have found in step one (if LSANormal.dot is NOT there – use your search function to find it and then copy and paste it into this Startup folder)
- In Word, look under Tools>Templates and Add-ins and check that “LSANormal.dot" is checked

Can I download the forms as Word documents?
No. The Word forms have been designed to work as templates (ie dialog boxes will guide you through the forms) and when you complete the forms you can then save them as a document - with all your detail added (You can later amend these documents in the areas you have filled out). They are not designed to be used as a document to fill out from scratch. The PDFs likewise cannot be saved as Word documents or to have data typed in. They are just to read or print.
Windows won't let me uninstall an old version of the templates.
There may be a permission error when trying to uninstall an old version. We recommend you log off the computer, log back on as the "Administrator" and try installing again.
Office 2007 - Addins don't show LSA Template Forms.
You will need to go into Word options and activate the LSANormal.dot Add-in.
- Click on the word "start button"
- Click word options down the bottom of the menu
- Click on Add-Ins in the left menu
- Click on the manage dropdown menu at the bottom of the page, select Word Add-ins and click Go.
- Put a tick in LSANormal.dot and press okay.
- Restart Word and you should now be able to see the LSA Template Forms from the Add-Ins menu
Looking for LSANormal.dot, but can’t find it.
The LSANormal.dot is installed to your Microsoft/word directory.
On Windows XP the directory will be something like:
C:\documents and settings\<username>\ApplicationData\Microsoft\Word\Startup
And on Windows Vista probably something similar to: C:\Users\<username>\AppData\Roaming\Microsoft\Word\Startup
