Legal Aid Forms and Resources

What Options Do I Have for Getting Forms?

You can obtain forms by ordering pre-printed forms or downloading forms to your computer. Here are the options:

Note: the Word templates package has been updated following the changes to rates on 1 July 2009. All the forms are now available in a single package - you will need to uninstall any package(s) you currently have and install the new one. Please click here for the Word templates

• Pre-printed forms: Law firms, courts, community law centres and other agencies that provide legal aid assistance can now place orders online. Orders are normally fulfilled within seven working days. To order pre-printed forms from Baseline Management, click here

• PDF forms: This is the quickest option. You can download just the particular form you want and save it, or print the form and use it just like a pre-printed form. Most items are available as PDF files. Instructions about how to download a PDF file are included with the lists of forms.

• Word template forms: These take a little longer to set up for first use, but save time and effort thereafter. Templates make calculations easy for tax invoices, amendments and application forms. The templates are downloaded as a single package and installed on your computer, available for use at any time. A Word template form allows you to pre-enter details about you and your law firm, it automatically performs financial calculations, and can be saved as a Word document. You can edit the Word document further if necessary, save it for your records, and print it. Templates are available for forms numbered: 1,2,3,4,5,5b, 6,6b,7,9,10,11,13,15,16, 19,20.
