Word Template Forms
Word templates are a useful tool for listed providers to pre-enter some recurring information, perform financial calculations and to keep client records on file electronically.
IMPORTANTThe Word templates package has been updated following the changes to rates on 1 July 2009. All the forms are now available in a single package - you will need to uninstall any package(s) you currently have and install the new one. Please see below for FAQs and more information on downloading. Word templates are currently available in a package which will install the forms onto your C drive and add a menu of forms to your Word toolbar. If you are planning on installing to a network (ie other than a C drive) please see the FAQs The Word package includes the following forms:

The Word package
Before you start
Read this before you start downloading the package of Word template forms.
If you are an existing user, you need to first uninstall all existing templates. This will assist in a trouble-free install of the new versions. You then download the package of the latest versions. Note: You cannot keep previous versions as well as new versions, since the new templates will overwrite existing templates if they remain on the machine.
Technical requirements
You will need at least Windows 2000 or later and Microsoft Office Word 2000 or later to install. The forms are not available for Macintosh at present. Please note that this file size is 4.2MB and unless you're on a broadband internet connection, the download time may take approximately 15-20 minutes.
How to uninstall existing Legal Services Agency Word template forms
You will need to uninstall all existing Legal Services Agency templates from your computer's hard drive before installing the new ones.
- First close down all other applications - especially make sure Microsoft Word and Outlook is closed.
- Delete the existing form templates from your system. Do this as follows:
a. Click on "Start" at the bottom left hand corner of your screen. A menu will appear vertically. Select "Settings” and from the menu that appears, open "Control Panel".
b. From this menu select "Add/Remove Programs". A list will appear - click on "LSA Templates Version 8", and then click on Add/Remove button.
c. The Install Shield Wizard window will appear - click "Next"
d. From the menu that appears click on "Remove" then "Next" and then "Remove". This will uninstall the old forms ready for the new ones to be downloaded
How to install new Word template forms
You are about to install a package of the Agency's Word template forms onto your computer's hard drive.
- If you have previously installed the Agency's forms onto your system, you first need to uninstall these (refer to previous step).
- You will be asked to complete your contact details (so we can send you any updates). Complete this and click the "Download" button.
- You will then be asked what you would like to do with this file. "Save this program to disk" is the option you should select.
- A menu will appear with your directory on it, for you to select a location to save this file to. "Desktop" is an appropriate location. Then click "Save" and the file will download onto your desktop. You will see this occurring.
- Close all applications (especially Microsoft Word and Outlook) and click on the icon that should now be on your desktop "LSA Templates Version 9 " .
- This will open up the installation program and will guide you through the process.
- You will be offered the option of a "Typical" which will automatically setup a directory on your computer's hard drive and install these forms. If you would like to choose the location for these forms to be installed select "Custom" then click the "Next" button. On the following screen click the "Change" button to select the new location of the template files. Please note that selecting this option will mean that you will NOT have the forms available on a drop down menu.
IMPORTANT NOTE: Once you have installed the forms on your computer do not move them! When you run Microsoft Word it will be looking for them in the location you chose when you ran the LSA Installation file.
You may need to restart your computer to complete the installation.
Where to find your LSA Forms menu once installed
In Word 2007, you will find the LSA Forms menu under "Add-Ins" in your tool bar.
In prior versions of Word, the menu bar will feature "LSA Forms" as a Drop Down menu between "File" and "Edit".

To download the Form, right mouse click here and select "Save Target As". Then follow these instructions to put the file in the right place on your computer.
Windows 2000 keeps template files in:
Profiles\[UserName]\Application Data\Microsoft Templates
[User Name] if your computer has been assigned a name (e.g. your name)
If you are unsure where your User Templates are stored then:
From the Menu Bar select ToolsOptions
Select the FileLocations Folder
The directory will show alongside User Templates
Choose Modify if you wish to see the complete command line.
Windows XP

When you load a template or add-in, it remains loaded for the current Word session only. If you quit and then restart Word, the template or add-in is not automatically reloaded.
- Click the Microsoft Office Button , and then click Word Options.
- Click Add-Ins.
- In the Manage list, select Word Add-ins, and then click Go.
- Click the Templates tab.
- Under Global templates and add-ins, select the check box next to the template or add-in that you want to load.
- If the template or add-in you want does not appear in the box, click Add, switch to the folder that contains the template or add-in you want, click it, and then click OK.
Using a Word template form
The first box displayed when you open a Word template form will provide the following information:
Getting started …
- This template provides a tool to help fill out the form. Dialog boxes displayed over the template will prompt you for the details to be entered. As you complete each box the template performs any calculations and displays the results, as well as the details you entered.
- If you CANCEL from a Dialog box OR click the background document, the process will stop. You can either Save the data entered so far (from the Word menu click File>Save As) or you can close without saving and start again.
- If you have used the template before, some details will display automatically in the appropriate dialog fields(e.g. your firm’s name, address, GST number). This is to save you re-entering the data repetitively. You can overtype the data if you wish.
- To change a tick from one option to another, simply cut and paste.
- Provider levels of experience and forum/proceedings categories are calculated automatically after selection.
- When all details have been entered, SAVE the document (as in 2 above), which can then be updated or printed as you wish.
NOTE: You can amend any details within your document. BUT take care with any new data entered, as it will affect any calculations already performed. If you make changes to any figures, click the smiley face on your toolbar. If you are unable to see the smiley face, use the keys Ctrl A together, then F9. This will recalculate any new results.
